Patients: your medical records
This policy covers the website. If you become a patient, the health information in your care is protected by HIPAA and described in our Notice of Privacy Practices, which you receive when you join the practice and can request from us at any time.
What we collect, and why
When you request a consult. Your name, email address, phone number and what you tell us about why you're getting in touch. We use it to reply and arrange your consult. This website itself stores nothing you type into the form: your request is emailed to the practice and also kept in the practice's secure records system, where it starts the file we use to arrange your consult. Please don't include detailed medical information in the form โ health details belong in the consult, not the form. We keep these requests as practice records; if you become a patient, they may become part of your medical record.
When you join the newsletter. Your email address, and, if you choose to tell us after confirming, your state. We send a confirmation email first and add you only once you confirm. Every newsletter has a one-click unsubscribe link, and unsubscribing deletes your address from our list. Sign-ups that are never confirmed are deleted after 7 days.
Anonymous visit counts. To know which pages are useful, we count page views and clicks on our own pages, together with the broad region of your time zone (for example "America") and the name of the site that linked to us. We use no cookies for this, and the counts are not linked to you or to your device. They are kept for 400 days. If your browser sends a Global Privacy Control signal, we don't count your visit at all.
Server logs. Like most websites, our servers record the IP address, time and request of each visit to the form and counting endpoints, for security and to stop abuse. These logs are deleted after 30 days.
Spam protection. Our forms use Google reCAPTCHA to tell people from bots. It loads only when you start using a form, not when you simply read the site. When it loads, Google receives information about your device and browser, and Google's Privacy Policy and Terms apply.
Cookies
We don't set cookies on this website. Google reCAPTCHA may set its own cookies once it loads on a form.
Who we share it with
We share information only with the service providers that run this website and our email for us, and only for that purpose:
- Amazon Web Services โ hosting the website, its forms and the practice's records system, and delivering our emails.
- Google โ our practice email (Google Workspace) and reCAPTCHA spam protection.
We don't sell your personal information, we don't share it for cross-site or targeted advertising, and we don't use advertising pixels or trackers. We may disclose information if the law requires it, for example to respond to a court order.
Your choices and rights
- Unsubscribe from the newsletter at any time with the link in any email.
- Ask us what personal information we hold about you, or ask us to correct or delete it, by emailing hello@myradhealth.com. We'll respond within 45 days. (Patient medical records follow the rules in our Notice of Privacy Practices, which can require us to keep them.)
- California residents: we don't share personal information with third parties for their own marketing purposes.
- Do Not Track and Global Privacy Control: we don't track you across other websites, so we treat every visitor the same way whatever their browser's Do Not Track setting. We honor Global Privacy Control by not counting your visit.
Security
Everything you send us travels over an encrypted connection (HTTPS), and access to what we keep is limited to the people and systems that need it.
Changes to this policy
If we change this policy, we'll post the new version here with a new effective date. If a change is significant, we'll say so on the home page.
